Many types of shoes come with a specific width. In most cases, the shoe size is the nominal width of the shoe in centimeters. For example, if you buy a size 10 shoe, it's about 10 centimeters wide. The European size conversion is a little different; your foot width can vary by up to 0.5 cm, so if you need a 41/42 you may have to order a 41 and add the 37 (meaning that the whole boot would be inside your foot).
